Before using this Nomination Form, please contact the operator and makes sure she/he wants to be nominated. We suggest you refer prospective nominees to the Club website to review the ByLaws and Articles of Incorporation. You may want to note that our Club is unique in that it not only encourage CW activity but is internationally managed and supports CW education through donations to qualified organizations. Also, your nominee is welcome to consult the “Member Roster” to identify and contact other possible sponsors, make schedules, etc. When you have received the nominee’s approval, go ahead with the Nomination Form.
